Viewing your Medical Records
If you have online access to your medical records you may see information that is not in plain English.
This is especially the case for older information in your records.
This is because clinical teams often use technical clinical language and abbreviations.
Often, what is written in the medical record is only for clincial people to communicate with each other.
Historically the infomration was not intended to be viewed by patients. The language used may not have the same meaning that it has in ordinary conversational usage.
Please do not be concerned if there are elements that are worded in technical ways or uses that seem blunt.
Due to how the clinical software is built, we are not able to change medical records. Where there are edits, these are all fully auditable and nothing is deleted we cannot change this.
